Order Replacement Uniform
As you prepare to transfer to HCRG Care Group and join us in the Medway ICRS service on 1 October, we’re asking you to complete our uniform ordering form so we can make sure your uniform is ordered accurately and ready for you.
If you have already submitted your uniform requirements by email, please complete the form below. This ensures we have all the information we need to place your order, as some details may be missing from previous submissions. You need to complete this form once.
If you’re able to submit your order by Friday 11 September, your uniform should be available to collect at one of our Connect & Collect events on Thursday 17 September.
Orders submitted after 11 September will still be processed and your uniform will be available to collect as soon as it arrives with us.
