About Bath and North East Somerset Adult Community Health and Care Service

With over 58 services across Bath and North East Somerset, we have been providing community health and care in the area since 2016. We employ more than 1,300 colleagues in over 20 professions, working in partnership with the Council and NHS in a new, joined-up health and care service. We have provided a single system to integrate all health and care whether it is run by us or not, combined with investing in the latest technology to ensure people get well and stay well. From staff nurses all the way to a Falls prevention team, we have general and specialized teams that strive to give you the best care.

Services we offer:

Urgent Community Response (UCR) service
Community wellbeing services (including wellbeing courses and volunteer services)
Community wellbeing services (including wellbeing courses and volunteer services)
Integrated social care and LD team providing care and support for people with complex needs
Dedicated health visitors
Specialist nurse care, eg heart failure, lymphoedema and intravenous therapy
Community hospital inpatient wards
Minor Injuries Unit
Care Co-ordination Centre
A range of outpatient therapy services including physiotherapy
Reablement
District and community nursing team
