Welcome to Surrey and North East Hampshire and Farnham Community Health Services

We will now be providing community health services for both the adults and children of Surrey. On this page, you’ll find information about both our services and what they provide as well as some of the latest jobs we have available within each service.

About Children’s Community Health Services

From April 2025 we will be providing Children’s Community Health Services in Surrey and North East Hampshire and Farnham. These support the health and wellbeing of families, from before baby is born right through to age 19 or age 25 for young people with special educational needs and disabilities (SEND). The wide range of services include those that every family will use, such as health visiting, school nursing, and school-age immunisations, and additional specialist services, including children’s therapies, specialist nursing, and developmental paediatrics for children with a medical need.

As one of the leading providers of community health and care in the country and a proven track record of providing children’s community services across the country, we’re well placed to provide the highest quality of care and transform the lives of children, young people and their families across Surrey. From pre-birth, our services are there to guide and be a friendly face to families every step of the way. But we also want to empower our families to access self-help and take control of their own health where appropriate and along with our service, help to relieve the pressure on the wider health and care system across the county.

Ahead of the service joining us in April, we’ve now started to release some exciting new opportunities to join our team as part of this service with varying Bands and levels of expertise required, join us in transforming lives for the better. About Adult Community Services

Similar to our service in North Kent, we provide adult community care services throughout Surrey and North East Hampshire. This goes from door-to-door community nurses to wheelchair services. Our aim with such services is to ease the squeeze on the A&E departments and allow our service users to live life the best way they can using the latest technology and techniques to ensure that everyone that crosses our path gets the most efficient and modern care to change their lives for the better. ‘Community services’ is a blanket term for a whole range of services but put simply, it means that they are services that aren’t provided in a large hospital (secondary care) or classed as primary care (GP Practices and urgent care centres). 

Services we offer:

  • Community nursing and intermediate care 
  • Community night nursing 
  • Specialist practitioner services for Diabetes, Tissue Viability, Parkinson’s, Multiple Sclerosis and Continence 
  • Diagnostic and Treatment Centre 
  • Podiatry 
  • Speech and Language Therapy 
  • Inpatient therapies in community rehabilitation wards 
  • In-reach services within the acute hospitals and the community 
  • Outpatients service at Farnham Community Hospital
