You’ll be meeting people in the community, directly in people’s homes and on-site at a clinic to improve people’s lives and avoid unnecessary hospital admissions to empower service users to get well and stay that way.

Role responsibilities

Our AHP roles bring a variation of challenges on a day-to-day basis according to each service user’s needs. Some roles may have varying responsibilities unique to the area but here are the typical duties and responsibilities when you’re a Health Visitor with us.

Allowing time for rehabilitation

Our aim is to improve independence, health, and wellbeing by supporting people in their home environment and local community. We provide non-time limited interventions which are goals-led.

Working with partners to provide safe and effective care

To provide safe, effective, and efficient interventions working with all relevant services (internal and external) to ensure patient/carer needs are met as identified by assessment.

Clinical-led activity

To undertake all aspects of clinical duties as an autonomous practitioner.

Working as a team and maintaining caseloads

Be responsible for a clinical caseload working within a multidisciplinary team ensuring service users receive optimum level of rehabilitation within available resources.

Assessment and diagnosis

To undertake the comprehensive assessment and accurate diagnosis of patients including those with a complex presentation, using investigative, analytical and clinical reasoning skills.

Communicating with team

Communicate effectively with the wider team and case manage services users as appropriate.

Speech and Language Therapist

As a Speech and Language Therapist, you’ll manage your own delegated caseload independently across a variety of predominantly educational settings.

You’ll work with parents/carers, education colleagues, the multidisciplinary team and the wider children’s workforce to provide a co-ordinated approach to children’s speech, language and communication needs, as part of their EHCP provision.

Pharmacy Technicians

You’ll play an integral role within our prison service, which aims to ensure primary care is at the heart of our everyday role, you will assist in the delivery of a high quality pharmacy service to the prison population.

Our motivated teams are driven by our belief that if you are a prisoner, you should receive same level of healthcare that you would in the community. We know that improving prisoners’ short and long-term health reduces their likelihood of reoffending.

Podiatrists

Working as part of the podiatry team, you’ll be providing assessment and treatment for patients referred for assessment and intervention.

You’ll also be responsible for the comprehensive assessment, treatment planning, delivery evaluation and discharge of evidence-based podiatry intervention in appropriate locations, for example patients’ homes and other community locations.

Occupational Therapists

The purpose of this role is to provide specialist person-centred assessment, rehabilitation and management for people with various therapy needs including the promotion of health and prevention of secondary complications.

Physiotherapists

You will play an integral role within our Therapy Services which includes Adult Community Rehabilitation encompassing GP referrals and hospital discharges, Falls Prevention, Intermediate Care, Neurological Rehab and Urgent Rapid Response.

You will work with the service user, family, carer and their GP to manage and stabilise the service user, delivering a collaborative approach for avoidable hospital admissions.
